Full job description is included as an attachment: Main Purpose of the Job: To work in a patient facing role dealing with patients for review of medication, chronic disease management, medication monitoring and health promotion and to provide a personal, professional/clinical contribution as a member of the multidisciplinary practice team To promote the safe, rational, high quality and cost effective use of medicines with the surgery by working closely with Pharmacist Practitioner, the primary healthcare team and manager To support the delivery of local and national prescribing policy i.e. MHRA, NICE, QoF and PCN DES, etc. To provide pharmaceutical advice to other members of the practice team and to patients e.g. therapeutic use of drugs, use of unlicensed medicines. This could be face to face or remotely. To support the achievement of practice based clinical governance through peer review, practice meetings, audit, education/training of other members of the healthcare team To provide support to less experienced staff including the medicines management team and the admin team in the practice Share workload with the other Pharmacist Practitioners in their absence Support the practice with undergraduate medical and pharmacist training Undertake duties as appropriate to support and ensure safe, appropriate and cost effective prescribing
