Brook Street Social Care are now recruiting a dedicated housing support worker to work with our client in M12 specialising in housing and homelessness.
Responsibilities:
1. To assist the Service Manager in the delivery of training sessions/workshops in local housing and welfare procedures.
2. To attend monthly drop-in sessions at Community Mental Health Teams across Tameside.
3. To assist the Service Manager in attending hospitals across the wider Pennine Care Footprint to facilitate patient discharge to those in Out of Area beds.
4. To ensure all aspects of diversity and inclusion are upheld to a high degree.
5. To ensure all aspects of health and safety compliance are upheld to a high degree.
6. To book in clients weekly and provide support at ward and community drop-in sessions.
7. To key work or co-work a caseload of client cases either individually or with a Recovery Worker.
8. To write supporting documents to other support services to support clients in their housing applications and recovery journey.
9. To actively promote the service and build up partnership working relationships with other services and initiatives.
Working Hours: Monday to Friday 9:00 - 17:00
Support: 24/7 support from Brook Street Social Care.
