Job Overview
This post is Fixed term/Secondment for 12 months until 02/05/2026 to meet the needs of the service.
The postholder will be directly accountable to the caseload holder and will be responsible for assessing, planning, implementing, and evaluating nursing care in the community.
The postholder will be expected to work collaboratively with the District Nursing Team and other agencies to provide clinically effective care.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main Duties of the Job

We believe in developmental and training opportunities for our staff and therefore are currently seeking to recruit highly motivated, passionate Band 5 Community Nurses.
The successful applicant will need to have excellent communication skills with an ability to provide clinically effective care within the District Nurse Team. The Central Division of Betsi Cadwaladr University Health Board delivers District Nursing Services across the Counties of Conwy and Denbighshire, working in an integrated way with partner organisations to deliver high-quality evidence-based care in order to maintain patients in their own homes and communities, averting hospital admissions, empowering the patient towards self-management, and implementing seamless patient-centred care and treatment.
You must be able to travel between patients' homes both within towns and rural areas in a timely manner. This post is rotational over 24 hours.
