Job Description:

Responsibilities:
* Provide personalised support in a sensitive and confidential manner, according to the personalised support plan.
* Understand and follow care plans, seeking clarification as necessary.
* Participate in monitoring and reviewing Service Users’ care plans.
* Work professionally with all members of the Service user’s Multidisciplinary team.
* Promote self-care and self-management to achieve maximum independence.
This may include:
* Assistance with mobility, feeding, washing, bathing, and dressing.
* Management of neurogenic bladder and bowel care.
* Assistance with domestic tasks such as laundry and cleaning.
* Preparation of food and drinks, and assistance with shopping.
* Emotional support to increase confidence and motivation.
* Administer medication as designated in the care plan.
* Assist with rehabilitation activities as directed.
* Document clearly in daily records on the progress of rehabilitation programmes.
Communication:
* Assist the healthcare coordinator and clinical lead nurse by communicating all information.
* Work in partnership with the multidisciplinary team and report any changes in a Service User’s condition.
* Ensure safeguarding and equality and diversity concerns are reported.
* Maintain clear, concise, and legible Service User records.
* Use Libertatem Healthcare’s ‘On Call’ system for additional support.
* Always act in a manner which respects confidentiality.
An enhanced DBS check & full training will be provided prior to commencing work with our client, supported by our clinical lead nurse.
