Main duties of the job Provide clinical leadership for the implementation and review of the ICS's Population Health Management Strategy. Support the place teams to embed place-based PHM initiatives, in line with the ICS's Joint Forward Plan (JFP) and eight strategic ambitions, ensuring sustainable implementation across system partners. Champion the use of data-driven decision-making and proactive care models across the system. Align PHM initiatives with the Fuller Stocktake recommendations and the Core20PLUS5 framework to tackle health inequalities.
