Job summary
The post holder is responsible for the deliveryof basic practice nursing services care to the practice population. Supportedby senior nurses within the practice, they will deliver care within theboundaries of their role, focusing upon supporting patients to be healthy,monitoring of long-term conditions, health prevention and screening activities.They will work collaboratively with the general practice team to meet the needsof patients, supporting the delivery of policy and procedures, and providingnurse leadership as required.

Clinical Practice
1. Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being
2. Implement and evaluate individual treatment plans for patients with a known long-term condition
3. Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition
4. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
5. Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence based practice and national and practice protocols
6. Work with patients in order to support adherence to prescribed treatments
7. Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care
8. Deliver opportunistic health promotion using opportunities such as new-patient medicals
9. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions
10. Support patients to adopt health promotion strategies that promote patients to live healthily, and encourage principles of self-care
11. Assess and care for patients presenting with uncomplicated wounds
12. Support and advise women requesting information relating to family planning needs
13. Support and manage health needs of women presenting for cervical cytology consultations

15. Implement and participate in vaccination and immunisation programmes for both adults and children

18. Registered General Nurse
19. Current NMC registration.
20. Evidence of continuous professional development.
