Due to internal promotions, this is an exciting opportunity to join our community integrated palliative care team to help ensure we provide excellent palliative and end of life care to patients in their own homes.
The post holder will provide visible leadership to the Palliative Support Team and, in liaison with the Head of Service, be responsible for the coordination of care delivery for adults with palliative and end of life needs. The post holder will deliver and delegate high-quality nursing care to adults with end of life needs, providing time-limited episodes of care for individuals to prevent unnecessary admissions to hospitals, facilitate early discharge from hospital, and support individuals in their preferred place of care.

A Full UK driving license and access to a vehicle are essential requirements for this role.
Main Duties of the Job
1. To provide direct assessment, advice, and nursing care for patients with cancer or an advanced terminal illness and their families, who require specialist palliative care support, including drug administration and syringe drivers.
2. To develop strong links with local GPs, District Nurses, local hospices, and other allied health professionals.
3. To act as a specialist resource for community and primary care colleagues in the management of patients with specialist palliative care support needs.
4. Contribute to the development and review of policies, procedures, and protocols within the Trust related to palliative care.
5. Develop and maintain highly effective working relationships with relevant clinical nurse specialists, social care providers, local hospices, GPs, and other community health professionals to promote and sustain a multidisciplinary/multi-agency approach to planned, coordinated care.
6. Be able to use a variety of strategies and a range of advanced communication skills to relate effectively with patients (when breaking bad news, dealing with anxious/bereaved relatives) and those involved in their care.
7. To identify the need for and initiate the use of translator services as required.

Job Responsibilities
1. Work as an autonomous practitioner within a multi-professional team, supporting patients at end of life with complex and specialist palliative care support needs, recognizing own limitations and seeking support from senior members of the specialist palliative care team.
2. Work in partnership with GPs and consultants to ensure the provision of seamless specialist palliative care.
3. Prioritize own workload on a day-to-day basis.
4. To contribute to nurse-led assessment of new and follow-up patients where appropriate to do so, and delegate to other staff as appropriate.
5. Delegate and monitor workload to the Palliative Support Team.
