Communicate effectively with young people and their families to provide informed choice and personalisation, recording agreed care plans in their electronic patient record. You will work as part of the multi-disciplinary team to provide individualised plans of care for young people and their families You will develop and deliver training with the support of the specialist midwives, being knowledgeable and supportive of the continuity of carer national agenda. You will work in partnership with a multi-agency and multi-disciplinary team from health and social work to provide high standards of care for young people with varied complex social needs. You will act at all times in a manner that upholds the Trusts values and Nursing & Midwifery Professional Framework goals, working as part of a team to ensure that women and relatives receive excellent care with compassion. You will participate in data collection to demonstrate the positive impacts of continuity of carer for young people in improving health and pregnancy outcomes. Please refer to attached job description
