Scope of Responsibility Caring Hearts is a privately run care company providing a wide range of housing, care and support services for adults and older people in their own homes. As a Review Officer, the postholder will perform a wide variety of routine/custom tasks to support on-going delivery of services provided by different staff teams within our organisation. You will be responsible for reviewing and ensuring care is provided in accordance with best practices and legislative requirements, reflecting policies, procedures and agreed standards under the direction of the Care Coordinator and Service Manager. Your work will ensure that the support provided to service users enables them to maintain skills and personal interests whilst still receiving person-led care unique to the individual. The postholder will maintain skills at current level and undertake such training and development as required from time-to-time to maintain and progress knowledge. This position would require a flexible approach due to the ever-changing environment of social care and able to adapt quickly to change. Candidates need to be reliable and motivated to drive new strategies forward. This role is for you Essential Job functions Work as part of the Branch team to complete essential tasks that leads to quality care Prior to each service commencing with a new client, carry out an assessment of what the client’s care needs are and risk assessments for the service we are to provide Planning, creating and managing care plans/care needs assessments and reviews as required Planning and managing care plan reviews and quality assurance checks with clients when required Take part in multi-disciplinary reviews and liaise with different agencies Carryout professional observations and competency assessments of Carers on the field.
