The mission of Charles River Community Health (CRCH) is to partner with individuals and families so they can thrive and lead healthier lives by delivering comprehensive, integrated, and equitable primary healthcare. The Primary Care Provider will work with a diverse group of nurse practitioners, MDs, DOs, and physician associates, collaborating with a team to provide quality patient-centered care. Cultural competence is required as the Primary Care Provider will engage with a racially, ethnically, and culturally diverse patient population.
YOUR RESPONSIBILITIES
* Perform holistic, evidence-based medical care services for health center patients.
* Evaluate, treat, and care for patients; write prescriptions; provide preventive health care advice and management; assess psychological impacts of illness; treat or refer patients appropriately.
* Refer patients for specialty services and document according to health center procedures, following up on results.
* Participate in co-management of a patient panel with team-based PA or NP.
* Engage in training activities for students and other clinical disciplines at the health center.
* Contribute to the formation and evaluation of applicable clinical protocols.
* Manage a patient panel of a minimum of 1500 patients.
* Ensure continuity of care when patients utilize other healthcare facilities.
* Participate in staff meetings, including quality assurance and medical audit activities.
* Provide paid 24-hour on-call medical coverage for one week at a time, approximately 1-2 times per quarter.
* Work one evening (until 7pm) weekly; occasional Saturdays may be required.
* Collaborate with staff to meet ACO quality goals, DPH accreditation, and other regulatory requirements.
* Perform other related duties as assigned by the Medical Director or designee.
YOUR QUALIFICATIONS, COMPETENCIES, TRAITS
* M.D. degree and completion of a residency in family medicine, internal medicine, or pediatrics required.
* Board-eligible or board-certified in your specialty.
* Current Massachusetts medical and prescribing license as well as DEA.
* Ability to obtain privileges at affiliated hospital(s).
* Understanding of ambulatory family practice healthcare program administration preferred.
* Experience with Patient-Centered Medical Home model preferred.
* Knowledge of EMR and Microsoft Office systems required; familiarity with Epic desired.
* Ability to work at either our Brighton or Waltham locations as needed.
* Fluency in Spanish and/or Portuguese a plus.
* Excellent communication skills, particularly with diverse cultures.
* Strong commitment to promoting diversity, equity, and inclusion.
* Participate in continuing medical education activities as required for licensure.
* Passion for serving underserved populations in diverse settings.
