More detail about the role The role will be predominately based in the Community with opportunity for inpatient work too. The post holder will manage a caseload via clinics, home visits and phone appointments. They will support the patient through their journey, providing self-management guidance, symptom management, medication treatments, psychological support and palliative care. Our Heart Failure Clinical Nurse Specialist service also delivers a rapid access diagnostic clinic and virtual ward. This is a hugely rewarding job in a welcoming forward-thinking team.
