Co-ordinate with GP surgery for routine reviews: cervical smear screening, breast cancer screening, annual chronic health review, etc., Liaise with relevant health professionals e.g. dietician, optician, chiropodist, continence nurse, dentist, Tissue viability nurse, Continence team, Community diabetes specialist team, etc., Liaise with pharmacists and contribute to the medicines management committee. Support mental health nursing staff in managing minor injuries and wound care management. Maintain accurate, detailed reports and records. Consult and coordinate with Multidisciplinary team members to assess, plan, implement, and evaluate patient care.
