Job overview
The Specialist Dementia and frailty teams provide a comprehensive assessment, diagnostic and treatment pathway supporting patients of any age with dementia as well as older adults over 75 years of age, who have functional mental health needs with an associated age related frailty.
You will be responsible for assisting with the delivery of patient centred care under the supervision of a senior member of the team; providing practical support to service users in performing day to day activities, guidance and education, and in assisting in their management and care based upon individual needs under the guidance of a senior member of the team.
You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hours a day service for 365 days a year, working shifts where appropriate and operationally required.
The Specialist Dementia and Frailty teams are community based and it is essential that you are able to drive a car and have a vehicle for work purposes with valid UK driving license.
Main duties of the job
You will be working as a community support worker to assist the multidisciplinary clinical team, working in one specified pathway or across pathways in accordance with area / locality structures. You will be working with Service users in the community in their own homes or place of residence across the MID locality covering Chelmsford, Maldon and Braintree. Your role will include providing physical health care checks, emotional support, carers support and signposting to other organizations.

Detailed job description and main responsibilities
* Basic understanding of the nature of mental health conditions and how it can affect people’s day to day life.
* Basic IT skills i.e. Word and Outlook.
* Experience understanding/recording of service user activity on a Clinical information system.
* Understanding of working with people with mental health conditions and an appreciation of vulnerability.
* Working with senior staff undertaking a range of clinical duties e.g. providing practical support and guidance to service users.

* NVQ3 in Health or Social Care or equivalent experience
