We are seeking a dedicated Assessment Worker to be the first point of contact for clients with substance use issues. In this role, you will conduct initial screenings and assessments, helping to identify client's needs and facilitating their entry into our comprehensive care services. You will work with vulnerable individuals who may have complex needs related to domestic abuse, homelessness, re-offending behaviour, and poor physical/mental health.
Key Responsibilities:
1. Conduct initial screenings and assessments to determine client's needs and eligibility for services. 2. Gather comprehensive information about client's substance use history, mental health, and social circumstances.
3. Identify appropriate interventions, services, and packages of support tailored to individual client needs.
4. Collaborate with team members to develop and implement recovery plans based on assessment findings.
5. Carry out risk assessments and implement risk management strategies.
6. Perform proactive outreach, including home visits and community appointments, to enhance engagement and promote social inclusion.
7. Identify and address barriers to treatment, developing innovative solutions to enhance accessibility.
8. Ensure clients have regular and up-to-date copies of their support and safety plans.
9. Maintain accurate records of client's assessments, progress, and plans.
