Purpose of the Role:
To provide personal care and support to service users with complex and challenging needs, enabling them to have choice and control over their lives.
Responsibilities include:
Improving and maintaining the independence of individuals.
Supporting individuals to stay at home if desired.
Enhancing the quality of life.
Promoting health and wellbeing.

Key Result Areas:
Support service users with personal care, domestic tasks, medication, and health related activities in line with agreed personal care plans to meet social and emotional needs.
Alert managers to changes in service user needs and respond in emergency situations, liaising with colleagues, other agencies, and carers as appropriate.
Assist service users to maximize independence through engagement in social activities, working with local community networks, voluntary, and selfhelp groups.
Complete records of service user progress: Personal Service Plans, logs, and reviews.
Liaise with colleagues, other agencies, service users with a range of complex needs, and their families/carers.
