Join our clinical site and discharge team at PAHT.
We are expanding our clinical site and discharge team, as we know this will directly impact us delivering excellent patient care.
Your role will be integral to the effective and efficient management of the hospital site and be the key contact for operational/capacity related issues throughout the Trust.
Our team is made up of senior registered professionals who offer advice and support to the wards, medical and surgical teams within our Trust.
The clinical site and discharge team are responsible for managing our hospital capacity including:
* emergency admissions
* non-emergency planned admissions
* managing the flow of patients during their stay to ensure they receive the right care as their needs change
* discharge planning/transfer of care

Main duties of the job
* Authority to act on behalf of Trust management out of hours.
* To support and assist where required the Clinical Site Manager with the Hospital at Night duties.
* Responding to, leading, managing and coordinating emergency situations 24 hours per day.
* Work with the Head of Site Management to develop the site management service to meet the changing demands of the trust.
* Provide situational data on demand and capacity.
* Taking a lead role in implementing the business continuity plan for the Clinical Site Managers.
* To assist in the management of clinical and non-clinical emergency situations.
* Ensure adherence to Trust policies, procedures and current legislation, and the interpretation of these in out of hour’s situations.
* To provide a seamless service that ensures that the hospital is a safe and effective environment for patients, visitors and staff across 24 hours a day.
Person specification
Qualifications
* Registered professional to NMC or HCPC
* Operational Management experience and a good understanding of the drivers and constraints around effective bed management and formal line management of teams.
Experience
* In-depth experience working in an operational role and good clinical understanding
Skills
* Be able to attempt to resolve issues and queries locally
* Excellent Communication Skills and manage the workload and adapt to the changing needs and priorities of patients, colleagues and the bigger organisation on a daily basis depending on the pressure experienced
* To organise and prioritise work
