Job Title: Social Worker - Adult Front Door
We are seeking a skilled and experienced Social Worker to join our busy community front door team at Wigan Council. This is a fast-paced role within adult social care working under the Adult Front Door model of practice, employing an asset-based approach.
The Successful Candidate Will Be Responsible For:
* Conducting assessments to identify risks for vulnerable adults.
* Developing and implementing care plans tailored to individual needs, in collaboration with residents and their support networks.
* Conducting home visits and telephone assessments, and contact with residents to monitor their safety and well-being.
* Coordinating with external agencies, healthcare professionals, and community resources to provide holistic support to residents.
* Participating in multidisciplinary meetings to review resident progress and develop intervention strategies.
* Providing crisis intervention and support to residents experiencing immediate safety concerns.
* Maintaining accurate and up-to-date records of resident interactions, assessments, and interventions in accordance with regulatory standards.
About You: We're looking for someone with significant social work experience and crisis intervention within Adult Social Care. You will have a degree or equivalent in Social Work and be registered with Social Work England. You will be driven and confident at working in a fast-paced environment, with excellent communication and organisational skills.
