An exciting opportunity has arisen for a dynamic, motivated, Nurse or Pharmacy Technician to join Neath Port Talbot Medicines Management Team for Domiciliary Care.
The successful applicant will be part of the integrated, multidisciplinary Community Resource Team who provide support and education in medicines management, specifically in domiciliary and intermediate care settings. This role aims to improve patient outcomes and minimise harm, including avoiding unscheduled care, admissions, and delays to transfers of care. It is ideal for individuals interested in integrated working and are committed to developing and expanding their skills to achieve the highest quality care and outcomes for patients in their own homes.
Main duties of the job
The role includes home visiting providing medicines management interventions to patients within their own home, involving lone working without direct supervision. You will liaise with a wide range of Health & Social Care professionals to identify solutions for patients in a multidisciplinary network with a focus on promoting independence, prevention and early intervention. Delivering medicines management training to relevant staff groups will also be included, and participation in audit processes.
The post holder must be able to travel around the locality.
