An exciting opportunity has arisen to recruit an enthusiastic and motivated HCPC Registered Band 6 Speech and Language Therapist to join our innovative Care Home Intensive Support Team (CHIST).
CHIST provides Nursing and Occupational Therapy staff for targeted, time-limited support to specified care homes as prioritised through our local system escalation processes and this role is to be embedded in an aim to enhance the overall care which is delivered by the CHIST Service.
You will have specialist clinical skills in Speech and Language Therapy to enhance the health and wellbeing of care home residents providing hands-on clinical support, advice, and guidance based on residents' needs.
You must have a desire to work collaboratively with patients and key partners to facilitate the best possible patient experience and outcomes. If you are interested in making a difference to patients and families living with heart failure and possess excellent communication, organisational skills and a flexible approach then we would like to hear from you.
The role is community-based with remote working and you will be required to meet travel demands of the role by working across the county in care homes registered with Staffordshire County Council.
Main duties of the job
As a Speech and Language Therapist joining our service you will be expected to provide support and advice with personalised care planning, nutritional advice/signposting and navigating links with other Health and Social Care services.
As part of this role you will also be expected to:
* Have excellent interpersonal and communication skills and an ability to quickly develop effective relationships with care home staff.
* Undertake tasks concerning the day-to-day organisation and operation of the team, working with the team leaders as required.
* Be skilled in risk management and demonstrate clinical leadership skills with an ability to prioritise the support needed in a care home, and escalate any issues appropriately.
* Promote effective team working in care homes and have a willingness to share your experiences and knowledge with others, in accordance with evidence-based practice.

Essential
* Degree in Speech and Language Therapy or equivalent
* Registration with HCPC
* Post Graduate RCSLT accredited dysphagia course or equivalent
* Attendance at relevant Clinical Excellence Networks

* Modules at master's level
* Evidence of further clinical study
Experience
Essential
* Relevant experience to the role including assessment and management of acquired communication disorders
* Assessment and management of routine dysphagia
* Ability to manage time and caseload effectively
* Supporting service users, families and carers in dealing with the impact of acquired communication difficulties
* Working in a multidisciplinary team
