Job summary An exciting opportunity has arisen for 2 static band 5 Occupational Therapist posts in the medicine team in Cardiff and Vale UHB. The medicine occupational therapy team is committed to the Right Care in the Right Place at the Right Time and are proud to deliver dynamic intervention to supportadmission avoidance or safe discharge home. The team align with the Occupational Therapy Operational Strategy, Therapies Rehabilitation Model and the 6 Goals of Urgent and Emergency Care. The team has excellent links with community partners following a Home First Principle and D2RA workstream.These posts arepart of the frailty pathway valuing patient centred care. Post holders will be required to work a7-day rota.
