We are seeking a first contact physiotherapist to join ourexpanding team.
Working hours: 37.5 hours per week, full time (part timeapplicants will also be considered)
Annual Salary £46,148 £48,634 dependent on experience
Main duties of the job
We are seeking to recruit a motivated and experienced HCPCregistered physiotherapist with proven clinical skills. The successfulcandidate will be comfortable working autonomously as a first contactphysiotherapist. They will work in our 3 practices and have a named GP forsupport and mentoring within each practice. They will possess excellentcommunication and team working skills.
Main duties of the job
They will work directly with patients to assess and treat conditions, manage long-term conditions as well as promote self-care.
The FCP will:-
-Provide clinical expertise, acting as first-contact physiotherapist and making decisions about the best course of action for patients care
-Deliver programmes of supported patient self-management
-Request investigations when necessary to facilitate diagnosis and choice of treatment regime.
-Refer to CCG commissioned therapy and specialist services when needed
-Provide an advisory role to other members of the clinical team and support our educational processes
-Administer corticosteroid join injections and request imaging

North Care Network is a recently formed Primary Care Network(PCN). We consist of 3 like-minded North Lincolnshire practices based inBarton-upon-Humber, Brigg, Winterton and the surrounding villages. We areresponsible for the care of around 33,884 patients. Wehave benefitted from a first contact physio service in our practices for asubstantial period of time and wish to maintain this excellent service whichhas been highly valued by our patients.
Job responsibilities
The physiotherapist will be primarily based within thepractices that constitute the network but may need to work from other premiseswithin the local area as determined by the networks clinical director andmanagement team.
JOB PURPOSE
1. Provide clinical expertise, acting as first-contactphysiotherapist and making decisions about the best course of action forpatients' care (including in relation to undifferentiated conditions). Thiswill involve seeing patients, without prior contact with their GP, in order toestablish a rapid and accurate diagnosis and management plan.
2. Progressand request investigations to facilitate diagnosis and choice of treatment
regime,understanding the information limitations derived from these and the relative
sensitivityand specificity of particular tests diagnostic services such as x-rays andblood test and interpret and act on results to aid diagnosis and the managementplans of patients.
3. Deliver programmes ofsupported patient self-management, in ways that facilitate
behavioural change, optimiseindividuals' physical activity, mobility, fulfilment of
personal goals and independence,and that minimise the need for pharmacological
interventions.
CORE RESPONSIBILITIES
1. Takes professionalresponsibility as a first-contact physiotherapist, with high-level
decision-making andclinical-reasoning skills to assess, diagnose and triage of patients.
2. Manages a complex caseload(including patients with long-term conditions, comorbidities andmulti-factorial needs),

4. Accountable for decisions andactions via HCPC registration, supported by a
professional culture of peernetworking/review and engagement in evidence-based

5. Streamlines pathway of care byproviding a responsive service so that patients receive timely access to care.

7. Provides care which istailored to the individual needs. This would include, appraising
the impact of individuals'clinical status on their general health, well-being, employment status(including in relation to function, physical activity, mobility andindependence.
8. Supports patients to set theirown goals and be confident in their approach to self-management
9. Communicates effectively andappropriately with patients and carers complex and
sensitive information regardingdiagnosis, pathology and prognosis.
