Erewash North Social Work Team is seeking to recruit a part time and suitably experienced Social Care Practitioner to join our very busy team based at Mercian Close, Ilkeston.
The team provide a generic service to people aged over 18.. The service covers people living in the community and residential establishments.
The successful candidate will be experienced in adult social care and be able to demonstrate that they can work in a person centred way in accordance with current legislation and policies.
Core tasks will include assessment, support planning and creative signposting. You will provide support and advice to clients and carers and whenever possible draw on community resources. The successful applicant will need to work in a team setting, and be part of a busy area duty system. You will also be expected to contribute to safeguarding investigations as appropriate.
You will need to have experience and an understanding of assessing and reviewing service user needs in the community and residential settings. Working within budgets is essential and having knowledge of outcome focussed when completing assessments and reviews would be beneficial.
You will be expected to work in multi-disciplinary partnership way with other professionals, service users and their families and carers.
You will create person-centred and strengths-based assessments and support plans and commission services to enable individuals to meet their outcomes, promoting a home-first culture.
You will need the ability to travel and ideally have access to your own transport for which a mileage allowance is payable.
