Kent Community Health NHS Foundation Trust
We are looking for a skilled Nurse to join our Pathway 3 Service based around Maidstone and Tunbridge Wells.
Pathway 3 supports discharge to local care home for individuals who require a further period of assessment to determine the long-term plan of care when an acute hospital admission is no longer required.
We work in collaboration with the acute trust Discharge Liaison Teams, Kent County Council Social Services Teams, All Age Continuing Health Care, local community services and organisations including the voluntary sector.
Main duties of the job
In this role, you will be visiting patients in their homes and assessing their needs to support the development of a long-term care plan.
The community-based nature does mean that a driving licence with access to a car are essential.
Communication and compassion are fundamental to the role; liaising with partner organisations, patients, relatives, and carers is vital to ensure processes are understood and supported by all involved in the individual's care and assessment.
In turn, you will be joining an "Outstanding" rated Trust, with a commitment to patient care, our workforce, your well-being, and your development.
Additional Working Pattern: 8-6 (4 day weeks, including approx 1 in 5 Saturdays)
About us
Rated 'outstanding' by the CQC, we've got a passion for delivering high standards of patient care and excellent services to improve the health of our communities. To do this, we need outstanding people who share our values: compassionate, aspirational, responsive, and excellent.
We know a commitment to equity, diversity, and inclusion leads to better patient care and a happier workforce. We pride ourselves on being diverse and welcome applications from people with varied backgrounds, perspectives, and experiences.
As a flexible working friendly organisation, we want you to be able to work in a way that is best for us, for our patients, and for you. Talk to us about a flexible working arrangement that won't involve sacrificing time for personal interests or family commitments.
Job responsibilities
The job description and person specification give you all the information you need about this role. Please look carefully at the criteria in the person specification and tell us what you have done that shows you meet this in your application.
At KCHFT, we strongly encourage colleagues in clinical and non-clinical roles who have direct or social contact with patients to be fully vaccinated. This ensures your safety and that of your colleagues and the patients we serve.
