Castle Place Practice are looking for a Care Coordinator to join our busy admin and reception teams, providing support to our Patients, GP's, Practice manager and other members of the Primary Health Care team. This is an essential role ensuring patients receive the highest quality of care and helping to coordinate various work streams, providing a welcoming and friendly professional service to general members of the public and all staff and visitors to the practice.
Main duties of the job

Job responsibilities
Previous experience of working within the GP Practice or other health care provider would be desirable. You must be organised, self-motivated, forward thinking with excellent customer service skills. It is important that the candidate is empathetic towards patients using the Practice and can remain calm under pressure while dealing with patient queries, answering calls, and managing GP requests. You should be assertive, able to multi-task, and handle a high volume of patients in reception while responding to other demands. Knowledge of medical terminology would also be an advantage.
This role is wide and varied, and the successful candidate will work closely with patients and a multi-disciplinary team of clinicians to coordinate their healthcare needs, directing them to the appropriate service to ensure they get the most suitable care from whatever health or social care provider is appropriate.
You will also be the first point of contact for the care homes covered by Tiverton PCN and will build and maintain good working relationships with care home staff. You will be allocated a caseload of identified patients and be required to ensure that their changing needs are addressed by taking into account local priorities, health inequalities, and/or population health management risk stratification.
