Job summary
The Community Intravenous Therapy service works alongside a variety of specialist services within the community nursing services including District Nursing, Urgent Community Response Acute Hospitals and other specialist community services, with the aim of providing proactive, effective personalised care plan ensuring we have the right staff with the right skills in the right place at the right time.
The service enables our patients to stay at home, avoid a hospital admission and facilitates early hospital discharge.
An exciting opportunity has arisen within our Community IV Therapy team for an experienced Nurse to join the team as the Lead CNS, working across our unique service to help us make a difference to our patients. The post holder will be responsible for the overall management of the daily function of the service, both clinically and operationally, providing management and leadership support to the team members and working closely with the Lead for the Outpatients Antibiotic Team (OPAT). Your role will include supporting the Senior Matron responsible for the service and take an active role in the quality and governance agenda requirements of the service, including the oversight and response to incidents, complaints and patient safety agenda. The post holder will need to demonstrate current leadership and managerial experience and attained post graduate qualifications pertaining to this role.
Main duties, tasks & skills required
* To promote quality improvement in patient care with the Trust by acting as the lead clinical nurse specialist overseeing the Community IV Therapy Service to ensure safe administration and effective management of patients who are discharged on IV antibiotics and require intravenous medication and chemotherapy medication and support of their intravenous infusion lines.
* To provide support to the service Matron in the planning and delivery of the service requirements, working collaboratively with specialist clinical nursing and speciality teams and community partners to meet key service delivery objectives.
* To lead the effective management of a defined caseload, ensuring all patients are regularly reviewed to support ongoing treatment or timely discharge as appropriate.
* To communicate effectively with patients, families and carers as required, intervening to overcome barriers to communication, using advocacy skills when necessary to achieve effective outcomes for patients who may have sensory impairment, confusion and/or aggression.
* To provide line management support to the community IV nursing team.
* Lead and coordinate the delivery of training and competency assessments for community nursing staff.

Qualifications
Essential
* Registered Adult / Child Nurse on the NMC Register. (field dependent upon role)
* Evidence of post-registration CPD / Post registration qualification in appropriate area of speciality or professional knowledge supplemented by specialist experience and short courses to degree / masters level equivalent FHEQ Level 6 /7.
* Recognised mentorship course.
* Non-medical prescriber or willing to work towards (where applicable to role).
Experience
Essential
* Evidence and ability to revalidate as required by the NMC.
* Significant post-registration experience at Band 6 or above Clinical Nurse Specialist within practice related field to role applied for.
* Evidence of ability to professionally lead/manage a team of staff.
* Demonstrable understanding of the management responsibility for pay and non-pay resources associated with a clinical service.
* Evidence of delivering change management both personally and as a facilitator.
* Evidence of achievements and career development in current post and of ongoing professional development/competence.
* Demonstrate specialist knowledge underpinned by theory and CPD relevant to speciality.
* Experience of developing educational programmes and teaching/assessing and supervising others in a clinical setting.
* Experience in service development.
* Working in a multi-disciplinary and cross-agency work environment.
* Can demonstrate expert nursing knowledge with an ability to assess, plan, implement and evaluate specialist care delivered to patients and give advice and information to patients/relatives/care staff.
* Awareness of current relevant NHS policy context relating to the speciality.
* Has understanding/experience of the principles of safeguarding.
* Knowledge of corporate and clinical governance.
