Who Were Looking For: Experience & Qualifications: At least one year in a Care Coordinator role within a community setting, with an NVQ Level 3 qualification or equivalent. Skills & Personality: Strong communication, organisational, and problem-solving skills, with a genuine passion for providing high-quality care.
