Discharge to Assess (D2A) Overview
D2A has been operational in Moray since 2021 and is about to embark on exciting changes to blend operations with our START (Short Term Assessment & Reablement Team) colleagues to assist patients out of hospital earlier to be assessed in their own homes. This is an opportunity for a suitably qualified Occupational Therapist to join the Discharge to Assess (D2A) Team in Moray as a Band 6 Occupational Therapist.
D2A is a therapy-led team working over 7 days a week supporting the early discharge of Moray patients to home from hospital. Staff within D2A work one weekend in four. Patients are assessed in their home environment by the D2A team comprising Occupational Therapy, Physiotherapy, Nurse Practitioner, Generic Therapy Support Workers and Social Care Assistants who provide support on discharge with a rehabilitation focus to maximise safety and independence in activities of daily living. Focused, goal-orientated assessment aims to optimise patients’ independence, self-management and the promotion of health and wellbeing over a period of 2 to 6 weeks.
The post holder will be able to demonstrate post-graduate training and experiential knowledge and be willing to undertake further training. You will be clinically supervised by a Band 7 Team Lead Occupational Therapist.
You will be expected to contribute to the ongoing development and evaluation of the service under the direction of the Lead Occupational Therapist using quality improvement methodology, and building on existing evidence to inform the D2A service delivery in Moray.
