Social Worker - Adults Physical Disabilities Team - Gateshead Council

Gateshead Council's Adults Physical Disabilities Team is seeking a dedicated and experienced Social Worker to join our team. We are committed to providing high-quality, person-centered support to adults with physical disabilities, empowering them to live fulfilling and independent lives.
About the Role:
As a Social Worker in our Adults Physical Disabilities Team, you will play a vital role in supporting adults with physical disabilities to maintain their independence and well-being. You will:
Conduct comprehensive assessments: You will assess the needs of adults with physical disabilities, identifying their strengths, challenges, and aspirations.
Develop and implement individualised care plans: You will work collaboratively with individuals, families, and other professionals to develop tailored care plans, setting out clear goals and strategies to support the individual's independence and well-being.
Provide support and guidance: You will offer practical support and guidance to adults and their families, helping them to understand their rights and access the services they need.
Advocate for individuals' needs: You will champion the needs and rights of adults with physical disabilities, ensuring their voices are heard and their wishes are considered in decision-making.
Work collaboratively with other professionals: You will work closely with a range of professionals, including healthcare providers, occupational therapists, physiotherapists, and other agencies, to ensure a holistic approach to care.
Maintain accurate and up-to-date records: You will adhere to relevant legislation and policies, maintaining accurate and up-to-date records to support the care planning process.
Who we're looking for:
We are seeking a highly motivated and empathetic Social Worker with a strong understanding of the needs of adults with physical disabilities and a commitment to person-centered practice.
You will have:
A recognised social work qualification (e.g., DipSW, CQSW, BSC Social Work)
A valid Social Work England registration
Experience in working with adults with physical disabilities, including experience in conducting assessments, developing care plans, and providing support and advocacy.
Excellent communication and interpersonal skills, with the ability to build rapport and trust with diverse individuals.
A deep understanding of relevant legislation, policies, and procedures relating to social care for adults with disabilities.
