Job Description:
An opportunity has come up for an experienced Personal Advisor to work with young adults transitioning from care to independent living. The role involves building supportive relationships, assisting with education, employment, and housing, and ensuring statutory visit requirements are met. Hybrid working is available, with office duties required every six weeks. Candidates should have experience working with young people aged 18+ and the ability to manage complex cases effectively.
Responsibilities:
1. Deliver high-quality support to young adults during their transition to adulthood.
2. Prepare, update, and review pathway plans in collaboration with managers.
3. Facilitate access to services and ensure care leavers engage with opportunities.
4. Maintain accurate and up-to-date case records.
5. Support young adults with complex needs, including mental health and housing challenges.
Requirements:
1. Proven experience working with young people, particularly aged 18+.
2. Knowledge of care leavers' needs and associated legislation, including the Children (Leaving Care) Act.
3. Strong organizational and interpersonal skills.
4. Ability to work independently and manage complex cases effectively.
5. Willingness to travel and work flexibly to meet service needs.
