Job summary We're excited to offer 0.7 WTE (26 hours per week) physiotherapists/Occupational therapist a fantastic opportunity to advance their careers with Solihull Community Services and Intermediate Care. Our therapy team works across multiple locations providing vital services to individuals who need further assessment and rehabilitation after being placed in discharge-to-assess beds or returning home for continued care. We focus on patients with complex discharge planning needs following hospital admissions. This role involves providing therapy services in patients' homes and working closely with community services including the community therapy team. The role also involves being part of the respiratory on-call list (Physio only) for UHB hospitals. Working hours are 26 hours per week, with shifts including 7-3, 8-4, and 10-6, covering weekends and bank holidays. A full driving licence and access to a vehicle are required. To support our patients in their recovery journey and minimise the risk of hospital readmissions. To make the transition from acute care to the community as smooth as possible, ensuring timely discharges and efficient flow. To liaise with various agencies to ensure safe, effective and efficient discharges. To provide Physiotherapy/Occupational therapy services in the patient's own home or residence. To contribute to seven-day working programmes across the organisation. To be able to manage your caseload and work independently. To supervise students on placements. To appraise, supervise and support junior staff and therapy assistants. To initiate/participate in audits, research and quality improvement programmes.
