Clinical Nurse Specialist – Community Services Team
We are a patient-centred charity that delivers palliative and end of life care to those with advanced life-limiting illnesses, who live in Northwest Surrey. We care for patients and provide support to families and carers in the community as well as in our In-Patient Unit and Well Being Centre at the Hospice.

Clinical Colleague
Responsibilities:
* Provide a high level of clinical expertise, to support and facilitate individualised patient centred care to patients with life limiting illness
* Provide care and support for relatives and carers in collaboration with the MDT.
* Facilitate and support the implementation of evidence-based practice and contribute to the ongoing development of the service.
* Participate in a seven day per week operational service.
* Demonstrate leadership through clinical expertise and specialist knowledge, delivering high standards of personalised care.
* Identify needs through a holistic assessment and co-creating a care and support plan with people living with cancer and other life limiting illnesses.
* Independently manage own caseload of patients and associated responsibilities.
* Independently manage the role of duty nurse on rotation with colleagues
* Assist with the smooth running of the Community Team as a team lead in consultation with the Community Services Manager.
Minimum Requirements:
* UK Registered Nurse
* Minimum 4 years relevant post-registration experience.
* Experience in the supervision of junior staff/others
