Band 6 - Senior Community Nurse - Tower Hamlets, London

In the District Nursing team, we have an ambitious vision for the Tower Hamlets community health service. We strive to deliver high standards of care at the point of entry into our services, aiming to improve access to the quality of life for all of our patients. One of our main aims is also to enable self-care and independence.
Our staff are central to delivering the service vision, and we are keen to recruit high-caliber staff who will be offered the support required to fulfill their roles, as well as the training and experience to further their skills.
You will be a strong team player, an excellent communicator who is adaptable, committed, well-organized, and able to work well under pressure. Having sound clinical assessment skills and the ability to effectively manage time is essential for this post.
1. To work as part of a team ensuring all referrals are triaged and have a timely visit scheduled.
2. To ensure all new admissions to the Community Nursing services have a clinically robust assessment and patient-focused care plan.
3. This is borough-wide cover.
4. To introduce and enable self-management and self-care.
5. To work as part of a team ensuring all patients have a safe handover to the professional and team to complete the care plan and measure the outcomes.
6. To work in partnership with the locality teams and partner providers to maintain effective patient flow.
7. To work within the financial resources for the patients and the service.
8. To work shift patterns on a rota: (8am-4pm, 10am-6pm, and 12pm-8pm) Monday to Sunday.
