We are seeking a dynamic Clinical Nurse Specialist who has a passion for frailty, to join the Frailty Team based within Same Day Emergency Care at Russells Hall Hospital. The main priority will focus on implementation and operation of Frailty Virtual Ward within the community in Dudley. The successful candidate will join the integrated team in the delivery of quality care for people and will be integral in enabling patients to achieve optimum function and receive appropriate treatment within their own home to prevent unnecessary admission to hospital.
