An exciting opportunity has arisen for enthusiastic and forward-thinking individuals to join our friendly and experienced team.
We are seeking motivated, passionate, dynamic, and flexible experienced Children's Nurses to work with children, young people, and their families throughout the Bradford and Airedale District in a range of community settings.
Our service provides nursing support for children and young people with complex and ongoing health care needs and conditions and their families wherever they may require it.
Our Continuing Care Team also delivers packages of care to these children to support them being cared for within their own homes.
We offer bespoke training and mentorship to enable development of skills and competence required for this post.
The service operates 08:00-20:00 (Mon-Fri) & 08:00-13:00 (Sat/Sun).
A full valid driving license and access to a car to use for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).

Main duties of the job
1. To assess the child's nursing needs, plan, implement, and evaluate care providing support & training in the community setting, aiming to prevent hospital admissions, reduce the length of hospital stay & support the ongoing management of care at home, by following agreed clinical pathways, policies & procedures.
2. To carry out initial and ongoing assessments for a defined caseload of children and young people with complex healthcare needs and conditions who require nursing care in a community setting.
3. To coordinate the workload as part of a skilled team, providing a quality-based nursing service for children, young people, and their families.
4. To ensure the child and family are at the heart of everything we do, ensuring patient experience is the best it can be.
5. To co-ordinate the workload of the Team on a daily basis, providing leadership to junior staff.
6. To act up in the absence of the Team Leader.
7. To provide a 7-day service for children, young people, and their families with a commitment to provide 24-hour cover to facilitate ambulatory/community care.
