Job summary An opportunity has arisen for a Practice Nurse to join us, ideally with Practice experience however for the right candidate, we are prepared to invest in their training. We are looking a full time position with consideration to part time or job share and the opportunity to support a newly qualified nurse. The position would also include on a rota basis, cover for extended hours which is currently Tuesday evenings, open until 8pm. Please note - Interviews will be held on Tuesday 11th February 2025 between 9am & 1pm Main duties of the job The post holder is responsible for the delivery of general practice nursing to the whole practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long term conditions, and the provision of preventative health care to the practice population. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. They will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures, and provide nurse leadership as required. The post holder will ensure that an equitable service is delivered to all patients including housebound patients.
