Key Accountabilities: 1. To provide evidence based personalised specialist palliative care without supervision, using an appropriate assessment tool. Recognise and review the physical, psychological and spiritual needs of the patients. 2. To provide continual assessment, planning, implementation and evaluation of evidence based, individualised patient care. Maintaining accurate clinical observations of the patient and acting accordingly, giving clear feedback to patients and family whilst working collaboratively with the multi-professional team. 3. IPU specific:- co-ordinate and lead a shift as the appointed Nurse in Charge; co-ordinate the IPU Team, including volunteers, to best meet the evolving needs of patients. Follow the Nurse in Charge handover, ensure adequate staff levels for all shifts, and subsequent reduction in staff levels if requirements reduce. Liaise with Reception and the Catering teams with regard to new admissions, discharges and RIPs. 4. WAH specific:- manage the caseload and liaise with MDT in the absence of the Senior Community Nurse and Lead Nurse. 5. Communicate effectively with patients and family members, providing information, support and advice in relation to the care of the patient, ensuring patient wishes and needs are paramount. 6. Communicate effectively with multi-disciplinary health care professionals, medical and social work colleagues, carers and managers across organisational boundaries. 7. Take on additional roles and responsibilities that reflect the changing nursing role in line with the NMC Standards for Practice, to ensure efficient care can be provided to address the needs of the patient. Actively support and engage with service development. 8. Engage with supervision to reflect, develop clinical skills and enhance your emotional intelligence. Attend regular 1:1 meetings as appropriate. 9. Ensure your mandatory training is current. Engage in educational activity to develop professional development to meet the revalidation requirements in line with the NMC guidelines. Demonstrate clinical competency in line with the Hospice competency framework, ensuring your clinical practice is up to date. 10. Participate in quality initiatives through active engagement as a champion, audit activity, facilitating education, policy writing and other tasks delegated as required of you. 11. Report incidents or concerns in a timely manner using the Hospice incident reporting system. 12. Actively engage as a practice supervisor and/or practice assessor to develop professional skills of students, whilst providing effective feedback. 13. Ensure the co-ordination and discharge of patients from IPU using the Discharge Bundle. 14. Maintain a safe environment when caring for patients, ensuring infection control procedures are adhered to, and risk assess patients daily to reduce the risk of patient falls on IPU. Complete all risk assessment weekly or if there is any change in patients condition/needs within WAH. 15. Actively encourage patients and their visitors to provide feedback using the I Want Great Care process.
