Main Duties & Responsibilities:
1. Develop, implement, and evaluate individual care plans under the appropriate delegation and guidance of a registered professional.
2. Monitor the patients' progress, make treatment decisions while assessing and re-assessing the patients' progress.
3. Take responsibility around risk management, providing feedback on concerns, and manage risk and safety in groups or other settings.
4. Educate patients and carers, empowering them to take positive steps towards improving their health and wellbeing.
5. Provide advice on the importance of exercise and other aspects of life to encourage healthy living.
6. Demonstrate understanding and application of professional, local, and national guidelines and legislation relating to health and social care provision.
Responsibility for People Management:
1. Demonstrate own duties to new or less experienced staff and participate in induction and support programmes.
2. Assist in the recruitment of relevant grades of staff as required.
3. Contribute towards developing a culture of learning and high-quality learning environments.
4. Support and supervise junior members of the team as required.
Responsibility for Financial and/or Physical Resources:
1. Use and maintain resources efficiently and effectively.
2. Exercise personal duty of care in the safe use and storage of equipment.
3. Use electronic resources to input, save, and retrieve information.
Responsibility for Administration:
1. Prioritize workload effectively, managing your own diary of appointments and visits.
2. Undertake any non-clinical/clerical duties as necessary.
3. Participate in audit as part of service review and improvement processes.
