Job Description and Person Specification
We have exciting opportunities for Permanent, Full Time, Senior Practitioner roles within our Living and Ageing Well Service in Adult Social Care.

About the role:

Whether you’re an experienced Senior Practitioner looking for career development in a role that offers a huge amount of learning and development opportunities, or a Social Worker looking for their first Senior Practitioner role, this the job for you.
We need agile leadership at all levels to embed a positive learning culture. Our Senior Practitioners assist the Team Manager with the management and development of the team and provide practice and professional leadership, supervising up to 6 staff and holding a small case load.
The following posts are available:
Hospital Discharge Team Senior Practitioner
This position requires working on weekends (one weekend every month) and includes a 10% allowance.
The team is dedicated to supporting the safe discharge of Southampton residents from acute hospital settings through collaborative problem-solving and close partnerships.
City Wellbeing Senior Practitioner
The City Wellbeing Team provides support to adults living in the community who have a physical disability or a diagnosis of dementia.
Adult Social Care Connect Senior Practitioner
Adult Social Care Connect (ASCC) serves as the first point of contact for all new SCC Adult Services referrals and inquiries.
About you:
To apply, you must have :
* Relevant degree / diploma in Social Work and have current registration with current Social Work England Registration
* At least two years post qualifying experience
* An essential post qualifying qualification such as Practice Educator, Best Interest Assessor etc.
