Staff Nurse Acute Response Team/Community Nursing Service
An exciting opportunity has arisen for committed and enthusiastic full time/ part time Registered General Nurses to work as part of the Carmarthenshire Community Nursing/Intermediate Care Service, based in the Acute Response Team.
The Acute Response Team is a multi-disciplinary team that provides a wide range of interventions in a patient's home.
Main Duties of the Job
The post-holder will be responsible for providing acute nursing interventions to patients within the community setting, thereby reducing the need for hospital admissions. The post offers the opportunity to develop your skills and experience whilst making a difference to patient care.
The post-holder will need to demonstrate that they have good communication skills and it is essential that you possess the skills to work as part of a team. The post holder must be able to work autonomously, which is needed to undertake a lone worker role in the community. You must be able to take on the challenges of a changing, modern-day multi-disciplinary Intermediate Care, Community Nursing Service.
You will be expected to work from one of two bases at Eastgate, Llanelli or Glangwili General Hospital, Carmarthen.
Various full and part-time hours are available.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

£30,420 to £37,030 a year per annum (pro rata if part-time)
