We are looking for motivated and enthusiastic Registered Mental Health Nurses, Occupational Therapists, and Social Workers to work in the Working Age Community Mental Health Service, supporting service users' recovery within the community. You will be recovery-focused, have good risk assessment skills, and possess a good range of therapeutic interventions supported by evidence-based practice.
Our Community Mental Health Teams operate Monday to Friday, with core hours being 9 am to 5 pm, offering flexibility through a hybrid model of working and flexible arrangements. We require motivated individuals with good initiative and organizational skills. You will work as part of a multi-disciplinary team providing interventions for working-age adults, ensuring that service users' recovery goals are identified and central to their formulation and care planning needs.

With access to appropriate clinical and management supervision, the post holder will develop and demonstrate a range of appropriate skills, including assessment (including risk assessment), formulation, safety planning, and clinical interventions.
The role provides the opportunity to coordinate the care of service users throughout their engagement with the service and any wider services required. The post holder will acquire all necessary skills to carry out the role, including practicing care coordination skills through shadowing opportunities and supervision. Workload will be negotiated through supervision and case load management with the appropriate clinical team managers.
You will work as a Care Coordinator and be an integral part of the Community Mental Health Team, undertaking shared roles and responsibilities such as initial assessments, care coordination, and team duty. Additionally, you will be required to carry out specific assessments and interventions, develop and deliver complex service user-focused Care Plans, and provide excellent packages of care, supported by a Multi-Disciplinary Team and Formulation process. You will also be expected to demonstrate leadership within your role, providing Clinical Supervision, Preceptorship, and support to junior colleagues. Student education and support are inherent to this role.

The post holder will acquire all necessary skills to progress into the role of care coordinator, practice care coordination skills in the clinical setting, seek out and engage in management and clinical supervision with senior clinicians and clinical team managers, establish priorities in workload in partnership with senior team members, and ensure effective use of time and resources. They will maintain a commitment to person-centered care and promote the use of the recovery model where appropriate, support and carry out comprehensive holistic mental health assessments, formulation, safety plans, and the planning and implementation of appropriate care and outcome measures, and act as a positive role model, providing support to other team members.
