St Luke's Hospice in Basildon has a fantastic opportunity for a community nurse team leader, on a part-time basis for 22.5 hours per week. You will manage a team of community nurses ensuring the provision of high standards of care to patients and their families or carers who have a palliative care need. Main responsibilities include: Providing leadership and guidance to the team in Hospice Community Services. Supporting the teams to ensure the delivery of the right care at the right time to individuals with a palliative care need in the community. Working, actively supporting and liaising with other health care professionals and agencies. Assisting in audit and research projects, particularly within the service, taking a lead on specified projects as appropriate. Monitoring and evaluating team performance, providing guidance and support where necessary. Fostering a culture of continuous improvement and professional development within the team. About the Candidate All applicants should have the following qualifications, registrations, skills and experience: A relevant nursing degree. Current NMC registration with an active PIN.
