To undertake comprehensive specialist nursing assessments that take into account the nature of a persons learning disability and its implication for care. To develop written and accessible Health Action Plans (HAPS) based on the information gathered from comprehensive health assessments, ensuring that these plans are recorded fully for each client and identified health needs addressed. To assess, advise and contribute to meeting the complex needs of people with mental health issues in addition to their learning disability by liaising and working collaboratively with specialist mental health services. To support people who are referred to the team for engaging in behaviours that challenge (e.g. physical aggression to others, self-injurious behaviours etc.); this will involve assessment of presenting behaviours, and developing formulations and comprehensive written Positive Behaviour Support Plans in line with NICE Guidance. (Additional qualification in PBS accreditation may be sourced) To support the teams acute liaison nurse in the provision of services to people with a learning disability admitted to Croydon University Hospital. To participate in Learning Disability Awareness training as part of the CLDT commitment to Croydon Health Services (CHS) staff to deliver generic and bespoke training sessions. To work with the community learning disability nursing team to meet the teams post-hospital discharge pathway target. To work jointly with colleagues in other disciplines (e.g. psychology) to carry out dementia assessments and autism assessment through the relevant team pathways. (Additional training including ADOS may be sourced) To contribute to multi-disciplinary planning and clinical meetings to coordinate care for people with a learning disability referred to the team. To promote well-being through activities, group work and training including womens and mens groups, healthy eating and lifestyle training and workshops. To retain legible, professional records, both written and electronically in the agreed format, and to maintain confidentiality, taking into account report writing standards and record keeping policy. To be professionally accountable for the effective and efficient management of ones caseload taking into account the need to combine own responsibilities and those of the team and adhering to professional codes of conduct reviewed at monthly clinical supervisions.
