The post holder will be based within the Community Mental Health Team for Older Adults (CMHTOA) in Huts 3 & 4, Falkirk Community Hospital.
As part of the Older Adult Community Mental Health Team the post holder will be supported by the team to carry a caseload of people experiencing a variety of mental health issues and dementia, to maintain the person within their own homes.
The post holder will work across the Falkirk Health and Social Care Partnership to provide a comprehensive, multi-disciplinary community mental health service to patients and carers in partnership with other agencies.
We are seeking a dynamic and forward thinking Health Care Support Worker. Experience working in older adult is essential. Experience working within a community mental health setting would be advantageous.
Ability and willingness to travel throughout the area of service delivery is essential for this post.
