Job summary To undertake a range of nursing assessments and provide appropriate care and treatment in conjunction with the GPs, Nurse Practitioners and other Practice Nurses/Healthcare Assistants or independently, in line with competence and need To maintain or improve standards of care and be actively involved in identifying practice population needs To demonstrate and apply knowledge related to the Quality and Outcomes Framework (QOF) and essential coding To provide therapeutic interventions within a broader health promotion/public health strategy and enable patients to take action to meet their priorities for health To help provide education and training regarding chronic clinical conditions as required To provide specialist advice/chronic disease management to patients and relatives To undertake any further training required to ensure appropriate role development To practice within the NMC Code of Professional Conduct and maintain accurate records and timely updates Main duties of the job Meet patients' nursing care needs and initiating and maintaining health screening and promotion clinics for identified areas of practice population, aware where modification or referral is necessary Manage nursing care of patients with chronic disease implementing recall systems where necessary utilising computer systems for inputting information Maintain efficient liaison with all Team members and other agencies to ensure continuity of care and assist with support of learning for all staff including trainees Infection control: Support patients and colleagues regarding IC measures according to local and national guidelines Emergency management: Be competent in anaphylaxis and resuscitation techniques Assist in clinical audit and setting up and monitoring of standards of care About us Cheveley Park Medical Centre is a small, friendly practice located in the Belmont area. We currently have 5030 patients with a varied age range in a largely affluent area. The clinical team consists of one GP partner, 3 salaried GPs, Nurse Practitioner, Practice Nurse, 2 Healthcare Assistants. We are a training practice hosting Registrars, F2 trainees and Medical Students. We also have a Care Coordinator and Pharmacist attached to the practice, supported by Administration / Receptionist staff.As a small team, we work closely together to provide the best proactive care possible for our patients which often means having a flexible approach to their needs.
