Partnering Health Limited has an exciting opportunity for a Practice Nurse in our Special Allocation Scheme (SAS). The SAS provides GMS services for patients who have been referred from their own practice due to their conduct/behaviour. The service works to deliver care to patients whilst working towards their rehabilitation back into their own practice.
About the Practice Nurse – Special Allocation Scheme (SAS)
As a Practice Nurse you will be responsible for the provision of safe and efficient primary care appointments in line with Primary Medical Services GP Contact, with full access to patient medical records. You will provide a caring service to the practice patients, including treatment, preventative care, screening, and patient education. You will also support the delivery of QOF, CQUINs, locally commissioned services, coding consultations, and seek opportunities to ensure the delivery of these services. The post holder will take a proactive approach to healthcare, be accountable for their own CPD, contribute to continually improving the clinical effectiveness of the clinics, and ensure teamwork by respecting and working collaboratively with all colleagues.
Practice Nurse Key Responsibilities and Clinical Duties:
1. Provide Treatment Room services and others, dependent upon competency level, including but not limited to: wound care, cervical cytology, swabs, family planning such as contraceptive initiation and checks, Chronic Disease Management (Asthma, COPD, Diabetes).
2. Screening: Undertake opportunistic screening including BP, height, weight, BMI, urinalysis, smoking status, and alcohol intake.
3. Health Promotion: Offer education and motivation for patients to change unhealthy behaviour. Recognise a patient’s readiness to change. Develop and maintain health promotion material.
4. Infection Control: Ensure that the highest infection control standards are met.
5. Emergency Management: Be competent in anaphylaxis and resuscitation techniques.
6. Safeguarding: Be aware of recognizing the need for safeguarding children and adults, raising concerns appropriately.
7. Administration and paperwork: Arising directly from this caseload (record-keeping, other written communications with local GP surgeries as required) and ensuring all appropriate templates and coding is completed for QOF, LCS, DES.
8. Team meetings: Essential to the delivery of team-based care, contributing to discussion of clinical practice standards, developing protocols, mutual professional support for colleagues, and actively participating in meetings.
Practice Nurse Skills & Experience:
1. Registered with NMC / HCPC.
2. Experience working in primary care.
3. Due to the nature of the role, mental health experience is desirable.
4. At least three years post-qualified.
5. At or working towards degree qualification / or equivalent experience.
6. You must be competent at providing treatment room tasks such as immunisations, wound care, chronic disease management.
7. You will be expected to ensure that mandatory education is kept updated as required by the policies of the organisation.
8. Excellent communication and interpersonal skills – written and verbal (particularly listening skills).
9. Clear understanding of professional responsibility and accountability.
10. Good organisational skills and ability to manage and prioritise own workload.
11. Good clinical skills and documentation skills using clinical IT systems.
12. Be able to clear an enhanced DBS check.
