A fantastic opportunity has arisen for a Wellbeing Assessor to join the Front Door Team This team will look to support individuals, with referrals being made from the age of 18. As a Wellbeing Assessor you will receive and manage incoming referrals, undertake a range of assessments and reviews whilst ensuring a strength based approach, ensuring a focus on community assets in partnership with individuals, carers and other professionals and where applicable arrange appropriate support packages that address the eligible social care needs identified. This will require accurately maintaining client records and ensuring that these records are logged correctly on to the departments IT system along with strong report writing and communication skills. You will also be supported with regular supervision and performance management sessions. You should have relevant knowledge and experience in Adult Social Care either in a formal or informal capacity and be able to fully meet the requirements of the post and cover the whole geographic area of the City either by possession of a current and valid driving licence or by access to public or other transport.
