This exciting and varied role provides operational support to the Pharmacy Patient Services Team (Dispensary and Clinical ward based services) and will also include opportunities to work in all sections of the Pharmacy Department.
Although you will have the opportunity to work across different sections of the Pharmacy Department you will primarily be based within the Patient Services Team where you will support the dispensary workflow and be instrumental in delivering medicines optimisation at the patient’s bedside.
Within the dispensary, your main duties will include, undertaking all aspects of dispensing and checking inpatient, discharge and outpatient prescriptions including Controlled Drugs. You will also have the opportunity to be involved / lead on projects and audits.
At ward level, you will be supporting the multidisciplinary team by delivering medicines optimisation services, for example medicines reconciliation, managing medication supplies, discharge facilitation and patient counselling.
Training will be provided for those Pharmacy Technicians who are not already accredited for Medicines Optimisation or Final Accuracy Checking. The post holder commence as a Band 4 with progression to Band 5 upon successful completion of these accreditations. Applications from PTPTs who will qualify within the next 3 months are welcomed.

The main duties of this post are:
Dispensary
• Undertaking all aspects of dispensing and checking for inpatient, discharge and outpatient prescriptions including Controlled Drugs in an accurate and timely manner.
• Dispensing and checking of speciality prescriptions, for example Clinical trials, Renal, Haematology, HIV, Immunology, and Intravenous Immunoglobulin (IVIG), Dermatology etc.
• Participate in the destruction of Controlled Drugs.
• Use professional knowledge to ensure that such duties are performed in accordance with Good Dispensing Practice.
Ward Level
• Undertaking all aspects of delivering medicines optimisation services by performing medicines reconciliation, managing medication supplies, discharge facilitation and patient counselling.
• Supporting the multidisciplinary team.
Core
• Participate in the training and mentoring of Pharmacy Technicians, Pharmacy Assistants, Trainee Pharmacists, Pre-registration Trainee Pharmacy Technicians and the induction of new staff members
• Support other areas of the Pharmacy Department: Clinical Trials, Quality Assurance and Supply Services.
All duties will be carried out in accordance with Trust policies and with regard to legal and safety requirements.
Our core working hours are between 08:00 and 19:00 Monday to Friday. You will also be required to work as part of a weekend and bank holiday rota between the hours of 09:00 to 17:00.

Dispensary Duties:
• Maintain an active involvement with the technical aspects of dispensing duties to include the dispensing of inpatient, discharge and outpatient prescriptions including Controlled Drugs.
• Dispensing of speciality prescriptions, for example Clinical trials, Renal, Haematology, HIV, Immunology, and Intravenous Immunoglobulin (IVIG), Dermatology etc.
• Dispensing or checking of Controlled Drug requisitions for wards or departments.
• Recording all transactions using the pharmacy computer system to ensure stock levels are accurate and up to date.
• Supervise the ordering, packing and checking of compliance devices.
• Carry out dispensary-based duties as an accredited Final Accuracy Checking Technician (FACT).
• Ensuring compliance with WDA GDP (Good Distribution Practice).
• Participate in the destruction of Controlled Drugs.
• Answering routine queries from wards and patients.
Medicines Optimisation Duties
• On wards and outside units such as St Peters Hospice working closely with Clinical Pharmacy Team and Pharmacy Patient Services Leadership Team.
o Undertake medication chart checking in line with procedures and guidelines.
o Perform medicines reconciliation within 24 hours of admission.
o Ensure the correct and timely supply of medication.
o Ensure prevention of missed doses.
o Assess and approve patient’s own medicines for re-use.
o Liaise with ward, primary care (if appropriate) and pharmacy staff to ensure seamless care in the provision of medicines.
o Perform an accuracy check on rewritten charts.
o Planning discharge in liaison with ward staff and pharmacy.
o Facilitate early discharge by use of ‘one-stop’ dispensing.
o Ensuring the accuracy and appropriateness of medicines for use on discharge.
o Process ward-based discharges where appropriate either by dispensing or final accuracy checking screened TTAs.
o Provide information to patients and carers on their medicines.
o Ensuring appropriate storage of medicines on the ward.
o Communicating with the multi-disciplinary team.
o Review ward stock lists.
o Minimise waste and processing items of medication for return or destruction.
o Where trained: Perform clinical prioritisation to prioritise high risk patients and or high-risk medication.
• Contribute to the training of nursing staff to ensure awareness and adherence to Medicines Optimisation procedures.
• Participate in Controlled Drug audits as required.
Core Duties
• Undertaking all aspects of dispensing and checking inpatient, discharge and outpatient prescriptions including Controlled Drugs.
• Provide Medicines Optimisation services.
• Participate in the training and mentoring of Pharmacy Technicians, Pharmacy Assistants, Trainee Pharmacists, Pre-registration Trainee Pharmacy Technicians and the induction of new staff members.
• Contributes to the development of new ways of working and the implementation of new and revised policies and procedures.

• Maintain competency as a Final Accuracy Checking Technician.
• Maintain competency as a Medicine’s Optimisation technician.
• Undertake and record CPD relevant to personal and organisational needs.
• Act as a role model.
• Any other duties, which may be required by the Director of Pharmacy for the benefit of the Pharmacy Service.
