The position is primarily based with the Hailsham & Eastbourne Teams, but flexibility to travel to other locations as needed to support service requirements is essential. It is essential that that you have access to a car, and hold a full licence. We provide a seven day service to patients from 08.00-20.00 hours (including weekend working & Bank Holidays).
What you will be doing
• Assessing new patients for nursing interventions in their own homes, creating & evaluating care plans
• Undertaking skilled nursing interventions for a devolved caseload with support of the District Nurse & Clinical Leads
• Assisting patients & carers in achieving optimal independence using best practice & following national & Trust policies & guidelines

You must be able to travel independently, carrying equipment & car sharing as required. You must have access to a car and hold a current full driving license.
