Slough OPMH has an exciting opportunityfor aCommunity Psychiatric Nursewho wishes to join our dynamicHome Treatment team.You will work alongside a very experiencedclinical team and take responsibility for the assessment and implementation of intensive care and treatment in the home of people with mental health needs, as well as people with dementia.
Youmust have specialist knowledge of the care of older people with mental health problems and mental health crises. You willbe requiredto travel within the community for the performance of your duties.
The serviceoperates7 days per week, so flexibilitywith regard tounsocial hours/ shift patterns will berequired.
We are a friendly,dynamicand supportive team. You will be actively encouraged to develop professionally in your role within theteam, andhave regular access to supervision and peer support in your role.
• Toensure the correct application of Mental Health Clinical Risk policies and procedures.
• Todemonstratea good levelof knowledge in the assessment of the cognitive, psychological,physicaland social functioning
• To develop and implement appropriate individualplans of care and regularly evaluate these with the Home Treatment Team, patientand carer whereappropriate.
• To provide a fast and flexible response to care needs, responding to referrals in a timely mannerand providing intensive treatment (up to three times daily).
• To participatein individual and/or group sessional work designed to meet the emotional and psychological needs of patients.
• To be knowledgeable of all necessary resources within the community to assistin the implementation of care plans.
• To take responsibility for being an active team member by participatingin regular staff support meetings, review meetings and team building exercises.
• To communicate formally and informally with team members, other colleagues from the multidisciplinary team, social services, primary care team, voluntarysectorand carers.

For this special opportunity, you will require:
• Previous knowledge and experience of working in a care environment and with service users who have complex mental health needs
• Good interpersonal, communication, observation, and reporting skills
• Able to empathise, be supportive and sensitive to the needs of others as well as being able to cope with distressing circumstances
• Able to work flexibly on a shift pattern to cover CRHTT Service as required and the ability to travel independently between locations to fulfil the requirements of the position.
