The South/East Community Therapy teams, based at Mount Gould Hospital and Plympton Clinic, are looking for a Therapy Support Worker to join their team of Occupational and Physiotherapists.
Working under the guidance of the therapists, you will be integral to the delivery of treatments and interventions to our patients in their own homes. You will support patients to reach their rehabilitation goals which will support them to remain living independently.

Main duties of the job
Our Therapy Support Workers are vital in supporting our Physiotherapists and Occupational Therapists to deliver therapeutic interventions to patients in their own homes. We are looking for someone who can take responsibility for the work allocated to them, use initiative, and be able to communicate clearly through clinical note taking and verbal feedback to the multi-disciplinary team.
You will be required to autonomously visit patients in their own homes across the South and East of Plymouth delivering Physiotherapy and Occupational Therapy interventions.
Experience of working in health or social care is desirable but not essential, as training and support will be provided to the successful candidate. An excellent work ethic and a desire to help and support patients at home is essential for this role.
Job responsibilities
Clinical:
1. To provide a continuing element of the overall therapy programme/treatment plan to clients within the setting. This may include clients with additional physical health needs, physical disability, behaviors that challenge and learning disabilities.
2. The post holder will be involved with monitoring the patients progress, making some treatment decisions while assessing and re-assessing the patients progress.
3. The post holder will observe general behavior, functional ability, and response of patients and record the information liaising with the senior therapist/worker.
4. To undertake client interventions and reviews, regularly meeting the relevant professionals to discuss progress, and reporting back if any problems occur.
5. To seek advice and supervision from the senior therapist/worker within the team as necessary to ensure appropriate patient management and care.
6. To use knowledge and skills to engage patients in therapeutic activities to promote healthy psychological and physiological wellbeing, encouraging self-management techniques where able.
7. To prescribe basic equipment and minor adaptations under prescribers guidelines and with guidance, where required, from a senior therapist.
8. Deliver, collect and instruct in the use of equipment for patients as part of their treatment plan.
9. Report defects in equipment, taking action to ensure such equipment is withdrawn from service.
10. Be responsible for material stock and maintenance stock levels as required. Order equipment from the Community Equipment Service.
11. Where applicable, to participate in reviews / case conferences of individual patients and contribute to the evaluation of their progress and development of their goals.
12. To attend multi-disciplinary/multi-agency case conferences, liaison, meetings, handovers and patient reviews as necessary and appropriate.
13. To support and encourage patients, their relatives and carers in an active approach to regaining and maintaining maximal functional independence.
14. To be responsible for contributing to the planning, monitoring and evaluating of patient centred individual and/or group activities as part of a regular review of the service requirements if required within your team.
Responsibility for People Management:
1. Participate in induction and support programmes in relation to new staff and volunteers where appropriate.
2. To receive line management support and supervision.
Responsibility for administration:
1. Prioritises workload effectively, this could include scheduling your workday appropriately or managing your own diary of appointments and visits. This requires a high level of time management and organisation skills.
2. To maintain accurate, comprehensive and up-to-date client records and related documentation in line with clinical guidance and LSW policy.
Responsibility for people who use our services:
1. To form a rapport and professional relationship with patients, maintaining clear boundaries for effective working practices.
2. Respect patient privacy, confidentiality, dignity, wishes and beliefs minimizing any necessary discomfort at all times.
Person Specification
Qualifications Essential:
* Regulated Qualifications Framework (RQF) level 3 qualifications (e.g. A levels or T-Levels) or demonstrated ability and commitment to achieve a relevant level 3 qualification.
* Level 2 functional skills, or as a level 3 apprenticeship such as the Senior Healthcare Support Worker apprenticeship (or other level 3 qualification).
Skills Essential:
* Excellent time management skills.
* Excellent communication skills.
* Ability to travel around Plymouth and surrounding areas.
* IT skills.
* Fluent in verbal and written English.
